For some seniors, special circumstances may call for specialized nursing care. Sunrise Skilled Nursing & Rehabilitation at Stratford Court of Boca Pointe promotes emotional health and physical well-being when you are recovering from an illness, hospitalization or even changes in medications. Based upon our founding principles of dignity and respect, we provide 24-hour Skilled Nursing & Rehabilitative care that emphasizes comfort and compassion. Rehabilitation services are available from licensed therapists seven days a week. We also provide 24-hour licensed nursing care for residents whose needs are best met in a long-term care setting.
Sunrise CareConnect–an electronic health record system–can be found in most communities, enabling us to better coordinate care, reduce risk, and improve resident outcomes.
As a resident’s healthcare provider, you can be granted access to Sunrise CareConnect to track your patient.
Each Sunrise community has a Quality Assurance and Performance Improvement (QAPI) program and committee that analyzes trend data, and evaluates systems and processes to improve delivery of care and resident outcomes. Learn more at ReferSunrise.com.

Our Partnership with Heritage:
Our collaborative and trans-disciplinary approach to care ensures safe and effective transitions to the next level of care. Utilizing early discharge planning through a comprehensive home assessment and simulation of home environment, early and frequent patient and family involvement in the plan of care, along with self-management of the disease process, and what to do if a problem arises after discharge. We set patients up for success from the first day. That’s why we’ve partnered with the region’s leader in post-acute therapy services, HealthPRO/Heritage, to implement our comprehensive, patient-driven discharge planning model: Safe Transitions. We collaborate with Heritage and Home Health professionals to provide supportive care, including assessments, treatment and education for patients and their families. From assessment through discharge, we provide clinical interventions that deliver results and satisfied patients.
Nicole McDonald Testimonial
"On my first visit to Sunrise, I noticed the residents and staff looked good. They were all smiling. We are like a big family. I love people and the best thing is when they come in sick and you are a part of getting them better. A hug or a thank you makes me feel so good."

“Stratford Court embodies the mission to champion the quality of life for all seniors. We pride ourselves on the high quality of care we provide to our residents each day. At Sunrise, I enjoy working with residents during a time of need. It’s fulfilling to see them go from the hospital, where they may feel almost hopeless and like they have a long road ahead of them, to walking out of rehab smiling, looking and feeling like a brand-new person.
I am proud to work with wonderful team members at Sunrise. Our customer-focused approach has made us one of the premier communities for post-acute services in the area. If you ever need post-acute services, I encourage you to come by our community for a personal tour.”
